Sussex Community NHS Foundation Trust, is the main provider of community NHS health and care across Brighton and Hove, East Sussex, High Weald Lewes and Havens and West Sussex. The Trust works to help people plan, manage and adapt to changes in their health, to prevent avoidable admission to hospital and to minimise hospital stay.
The Trust is consulting on how they can improve the way they care for, treat and support patients over the next few years. The Trust wants to hear directly from patients about what they would like their services to look like.
- What do our patients need from us?
- What challenges are patients and staff currently facing?
- What do patients and staff want our services to look like in the future?
- What do patients and staff think of our progress so far?
The session will take place on Wednesday 6th July, from 12:00-15:00, at The 1st Central County Ground Hove, BN3 3AN. Teas, coffees and biscuits will be provided.
Potential attendees can express an interest in the session by completing this form by 5pm on Wednesday 29th June.
