Sussex-wide survey for carers of people living with Severe Mental Illness

East Sussex County Council are running a survey for carers of people with Severe Mental Illness (SMI). This includes people who have a diagnosis of schizophrenia or bipolar disorder, or who have a history of psychosis.

This survey is intended for people who live anywhere in Sussex who give support or care to a person with a SMI diagnosis. Attending an annual physical health check can help with prevention and early identification of health conditions, and link people with community organisations. By completing this survey, you could contribute to improvements to the way health and care services are delivered.
